The point is that it should have been reported at the time. Not reporting it allows for further transgressions (if that were the case) or a "metoo" attitude. Cannot hide the fact that it takes the focus off the earlier debarcle where no one is charged or found guilty but money is still paid to the "other" party.

Only a couple! The biggest fail point was the ego of the CEO and his view that innovation and risk was preferred to established, proven tested concepts rules and safety, because he "knew" his untested technology was best. Unfortunately a disaster waiting to happen.

A 5 inch thick carbon composite tube with titanium rings glued on either end. Would be like popping a Pringles container.

When we used to manufacture the GRP polyester resin/chopped strand/woven roving mix hatches for the Oberon class submarines (not associated with the pressure hull), the most important task was to ensure that there were NO air bubbles in the laminate (around 12mm thick). At the diving depths of the Oberons any air bubbles in the laminate could travel and delaminate the hatches. We did recommend a vacuum chamber to assist with air bubble removal (in the course of laminate layup/manufacture) but the design authority said that would be in contravention of the original design process (original build in Scotland).
